Registered Nurses or Enrolled Nurses to join our growing team as a Chronic Disease Management Health & Wellbeing Nurse.

This is an exciting opportunity to play a key role in preventative healthcare and chronic disease management, working closely with GPs, allied health professionals, and patients to improve long-term health outcomes through education, care coordination, and ongoing support.

About The Role

In this role, you will:

• Support patients to manage chronic conditions and achieve positive health outcomes
• Provide health coaching and promote positive behaviour change initiatives
• Coordinate care with GPs, specialists, allied health providers and community services
• Conduct and coordinate clinical services including ECGs, Spirometry, ABI testing and DMMRs where appropriate
• Maintain accurate patient records and ensure Medicare compliance requirements are met
• Proactively identify eligible patients for chronic disease management and preventative health services
• Build strong relationships with patients and provide ongoing education and support

About You

To be successful in this role, you will have:

• Current AHPRA registration as a Registered Nurse (RN) or Enrolled Nurse (EN)
• Previous experience in Chronic Disease Management, Care Plans and Health Assessments (highly regarded)
• Prior experience in General Practice or Primary Healthcare (preferred)
• Strong understanding of preventative health, health promotion and chronic disease management principles
• Excellent communication and interpersonal skills
• Strong organisational and time management abilities
• The ability to motivate and engage patients in improving their health outcomes
• A proactive, patient-focused approach to care
