What You Will Be Doing

As a Registered Nurse within the Hospital in the Home team, you will provide comprehensive clinical care to patients receiving hospital-level treatment in their own homes following a recent hospitalisation.

Working as part of a multidisciplinary team, you will undertake nursing assessments and deliver a range of clinical services including:

  • Comprehensive nursing assessments and clinical observations
  • Medication administration and reconciliation
  • Wound care and ongoing treatment management
  • Collection of pathology specimens
  • Patient and carer education
  • Liaison with medical officers, general practitioners and other healthcare providers
  • Monitoring patient progress and escalating concerns where required

This role requires a high level of clinical judgement, adaptability and independence while maintaining a strong focus on patient safety and quality care.

About you

We are seeking an experienced Registered Nurse who is passionate about delivering high-quality care in a community setting.

You will bring:

  • Current registration as a Registered Nurse with AHPRA
  • Extensive clinical nursing experience
  • Strong assessment and clinical decision-making skills
  • Excellent communication and interpersonal skills
  • Demonstrated ability to manage competing priorities and work autonomously
  • Confidence in providing care independently within patients' homes
  • A patient-centred approach and strong advocacy skills
  • The ability to quickly assess situations and respond appropriately to changing clinical needs
