What you’ll be doing
Client Intake & Assessments
- Receive and process referrals into appropriate services
- Conduct comprehensive home-based assessments and WHS screenings
- Identify risks and escalate where required
- Assist prospective clients to understand funding pathways
Care Planning & Coordination
- Develop tailored care plans aligned with the Aged Care Quality Standards
- Collaborate with internal/external providers to support independence and wellbeing
- Monitor quarterly budgets and support reassessments
- Provide guidance and direction to service delivery staff
Documentation & Compliance
- Complete accurate and timely client documentation
- Ensure compliance with legislation, organisational policies and quality standards
- Record hazards and escalate risks appropriately
Client Support & Communication
- Act as a trusted contact for clients and families
- Provide clear guidance across My Aged Care, Home Care Packages and Support at Home pathways
- Maintain respectful and professional communication at all times
Organisational Contribution
- Participate in a rotating on-call roster
- Promote a positive customer service culture
- Uphold Goodwin’s reputation as a quality care provider
What you’ll need
Mandatory
- Certificate IV or higher in Service Coordination (or extensive experience in similar role)
- First Aid Certificate
- Current driver’s licence & own vehicle with comprehensive insurance
Essential Skills
- Strong knowledge of ageing issues, including dementia and behaviour management
- Ability to undertake case management to a high standard
- Excellent communication, negotiation and problem-solving skills
- Strong organisational and time-management capabilities
- Competent in Microsoft Office
Desirable
- Experience in aged care, home care services or a related sector
