About The Role

The cohealth Medical team delivers holistic, client-centred healthcare across Melbourne's northern and western suburbs through a network of community clinics. GPs work within multidisciplinary teams, supported by nursing, allied health, administration, and specialist programs, to provide evidence-based care tailored to the diverse needs of the community.

Your Duties Include

  • Provide comprehensive, evidence-based medical care to individuals, families and communities
  • Deliver preventative, coordinated and ongoing patient care in a multidisciplinary setting
  • Support high-quality, safe healthcare through continuous improvement and best practice
  • Work collaboratively to improve health outcomes for diverse communities

Our Ideal Candidate

  • RACGP Vocationally Registered General Practitioner
  • Medical Practitioners Board of Victoria current Practicing License
  • Commitment to work within a community focused whole of population health approach, recognising the importance of providing a medical model within a holistic health response
  • Excellent interpersonal and communication skills
