Amplar Health Home Hospital is continuing to expand its Transition Care Service (TCS), creating exciting opportunities for Enrolled Nurses looking to grow their careers while maintaining and building their clinical skills. Whether you're currently working in acute care and looking for a change, or working in another nursing setting and wanting to broaden your experience, TCS offers the opportunity to apply your clinical knowledge in a rewarding sub-acute environment, supporting patients as they recover or improve their health outcomes. We are seeking Enrolled Nurses to join the team on a full-time or part-time basis (minimum 0.6 FTE).

- Monitor patients progress and report changes to a broader multidisciplinary team
- Maintain up to date documentation
- Contribute to care planning and patient assessments
- Support activities of daily living and administer medications
About you
You’ll enjoy a balanced 7-day rotating roster (mornings, afternoon and nights) offering variety, flexibility, and work-life balance, within a supportive team environment.
- 1 year + experience as an Enrolled Nurse
- Current unrestricted AHPRA registration
- Certification to dispense medication
- Experience in wound care management
- Hold a current CPR certificate
- High attention to detail with a patient focused approach
- Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain)
- If successful, willing and able to provide proof of up-to-date vaccinations
- Nurses with agency experience in acute care are encouraged to apply
