As a Care Coordinator, you'll use your clinical expertise to support members through care planning, education, discharge support and recovery management. You'll work closely with hospitals, GPs and allied health providers to reduce avoidable admissions and improve health outcomes.
Key responsibilities
- Develop and manage personalised care plan
- Provide clinical advice, coaching and education
- Coordinate care across hospitals, primary care and community services
- Support recovery pathways, medication, pain and falls management
- Contribute to quality, accreditation and continuous improvement
About you
- Must have relevant clinical experience
- Minimum 5 years' relevant experience
- Strong knowledge of the public and private health systems
- Confident, collaborative and able to work autonomously
Essential Qualifications
- Bachelor of Nursing (or equivalent qualification leading to RN registration).
- Current AHPRA registration as a Registered Nurse.
