We're looking for a compassionate and proactive Case Worker – Service Navigator to join our Connected Communities – Homelessness & Complex Care Team.

Working onsite at Park Towers, South Melbourne, you'll build trusted relationships with residents, providing practical support, advocacy, and service navigation that helps people improve their wellbeing, maintain housing stability, and strengthen their connection to the community.

About The Role

This is a unique site-based role where you'll become a visible and trusted presence within the Park Towers community. Working from a strengths-based, trauma-informed approach, you'll proactively engage residents, identify emerging needs, and provide early intervention, advocacy, brief case management, and coordinated support.

You'll assist residents to access health, housing, mental health, disability, aged care, welfare and community services while promoting independence, social inclusion, and improved quality of life.

Park Towers, South Melbourne

9am – 5pm Weekdays | Full-Time

What You'll Be Doing

  • Build positive, trusted relationships with residents through regular onsite engagement.
  • Provide assessments, brief case management and service navigation.
  • Support residents to access housing, health, mental health, welfare, disability and aged care services.
  • Advocate for residents and coordinate referrals across multiple service providers.
  • Identify risks early and provide timely intervention and support.
  • Facilitate community activities that strengthen social connection and wellbeing.
  • Collaborate with internal teams, housing providers and external agencies.
  • Maintain accurate case notes and contribute to coordinated care planning.

What You'll Bring

  • Tertiary qualifications in Social Work, Psychology, Community Services or a related discipline.
  • Experience supporting people experiencing homelessness, social disadvantage or other complex needs.
  • Strong case management, advocacy and service coordination skills.
  • Excellent communication and relationship-building abilities.
  • Knowledge of health, housing, welfare and community service systems.
  • Ability to manage competing priorities while maintaining a person-centred approach.
