Your New Role

As Wellbeing and Transitions Manager, reporting to the National Wellbeing and Transitions Manager, you will provide support to seamlessly transition current and prospective residents to Peninsula Grange in Mornington..

You will enjoy working within our Better Together social model of ageing well, which offers a continuum of care and support services for our Residents, within a range of accommodation options in one location.

What does your new role look like?

• Supporting both planned and unplanned transitions into Aged Care sector.

• Providing a meaningful and seamless experience for the residents and their families

• Maintaining and maximising occupancy levels; monitoring referrals/sales, ensuring targets are delivered and customer experience is upheld

• Engaging with local communities, promoting the Australian Unity Care Community

• Developing strong relationships with the co-located Retirement Community and wider teams

• Building strong relationships with Service Providers, Health Professionals, Community Groups, and other partners to support referrals

• Advocating our Better Together model and values of Bold, Warm and Honest.

About you

• You have a strong capacity to listen and understand our client’s needs and requirements

• Availability to work full time Monday to Friday

• Strong sales and communication skills with the ability to build working relationships and convert referrals/consultations

• Knowledge of Aged Care funding, experience in Residential Aged Care sales- beneficial

• Demonstrated experience in Aged Care is preferred

• Current Drivers Licence and reliable car

• All Australian Unity employees must be fully vaccinated for COVID 19 and must have received a 2026 flu vaccination
